This barely qualifies as a custom, but it’s worth showing how just a simple change can result in a “new” figure.
For my “Bat history” shelf, I needed a Jason Todd Robin. I researched what visual differences existed between him and the Dick Grayson version. Turns out, the only consistent difference was the curly-Qs in his hair. (Sometimes his memorial is drawn with a longer cape, but that seems to have been a ret-conned change.)
Thus I performed a simple head swap with the DC Direct modern Teen Titans Robin. The final element was painting the mask black.
And now the “history” shelf is that much closer to complete.

Moderate: New and modified parts.
Skilled: Lots of new and modified parts. “Frankensteining” often involved.
Advanced: All of the above, plus a great deal of original sculpting.
